Accident Summary:
THE AIRCRAFT WAS BEING USED TO PERFORM A KICK-OUT RESUPPLY MISSION TO A LOG SITE. AT APPROXIMATELY 1420 HOURS ON 18 JAN 71 THE AIRCRAFT WAS LOADED WITH APPROXIMATELY 1,000 POUNDS OF FUEL, 1,000 POUNDS OF CARGO AND TWO PASSENGERS. THE AIRCRAFT FLEW TO THE LOG SITE AND REQUESTED SMOKE WHICH THEY PROPERLY IDENTIFIED. THE AIRCRAFT COMMANDER MADE A VISUAL RECONNAISSANCE OF THE AREA AND STARTED HIS APPROACH IN A SOUTHEASTERLY DIRECTION, INTO A FIVE TO TEN KNOT HEADWIND. AS THE AIRCRAFT COMMANDER TERMINATED HIS APPROACH TO A HIGH HOVER HE NOTICED AN EXCESSIVE AMOUNT OF AFT LEFT CYCLIC WAS REQUIRED. THE ENGINE RPM DETERIORATED SLOWLY TO 6300 RPM. THE AIRCRAFT COMMANDER HAD BEEN UTILIZING 6500 RPM DUE TO THE FACT THAT THIS AIRCRAFT BUILT QUITE A BIT OF RPM DURING DESCENTS. THE AIRCRAFT COMMANDER APPLIED FORWARD CYCLIC AND REGAINED EFFECTIVE TRANSLATIONAL LIFT AND EXECUTED A GO AROUND. THEY MADE A RIGHT HAND ORBIT AND ACCELERATED TO 60-70 KNOTS. ANOTHER APPROACH WAS MADE ALONG THE SAME AXIS OF APPROACH AS BEFORE AND IN CONJUNCTION WITH THIS THE ENGINE RPM WAS BEEPED UP TO 6600 RPM. THE AIRCRAFT COMMANDER ADVISED THE CREW CHIEF TO TELL THE LOADMASTERS TO GET READY TO KICK OUT THE CARGO AS SOON AS THEY WERE IN POSITION. ONCE AGAIN AN EXCESSIVE DEGREE OF AFT LEFT CYCLIC WAS NEEDED. THE AIRCRAFT COMMANDER WAS UNABLE TO STOP THE HELICOPTER AT A HOVER. WHEN THE LOGMASTERS WERE ALERTED TO GET READY FOR THE KICKOUT THEY HAD UNBUCKLED AND MOVED FORWARD, WHICH AGGRAVETED THE FORWARD CG CONDITION AND CONTRIBUTED IN THIS CG CONDITION BEING WORSE THAN ON THE FIRST APPROACH. THE AIRCRAFT COMMANDER ATTEMPTED TO REPOSITION HIS LEFT FOOT TO ALLOW FOR A FURTHER LEFT CYCLIC MOVEMENT. THE AIRCRAFT WAS STILL MOVING FORWARD SLOWLY, THE RPM STARTED TO DROP AGAIN, AND THE AIRCRAFT STARTED INTO A SLOW UNCONTROLLABLE TURN TO THE RIGHT. THE AIRCRAFT COMMANDER STARTED TO APPLY FORWARD CYCLIC PRESSURE TO EFFECT A GO AROUND. THE AIRCRAFT STARTED TO SLOWLY SETTLE INTO THE TREES WHILE CONTINUING TO TURN TO THE RIGHT. THE AIRCRAFT STARTED TO SETTLE INTO THE TREES STRAIGHT DOWN UNTIL THE ROTOR RPM DECAYED TO THE POINT THAT THE AIRCRAFT NOSED OVER AND TO THE LEFT AND CAME TO REST AT ABOUT A 30 30 DEGREE ANGLE OF BANK.\\
