AT APPROXIMATELY ^1710^ HOURS ON ^11 FEBRUARY 1969^ ARMY HELICOPTER ^729^ LIFTED OFF OF ^VUNG TAU^ ENROUTE ^BLACK HORSE^ WITH A FULL COMPLEMENT OF CARGO AND CREW WITH TWO PASSENGERS ENTOW. JUST AFTER CLIMB OUT THE PILOT ^CW2 GILYARD^ GAVE THE CONTROLS TO THE CO-PILOT, ^1LT CASKEY^ SO THAT HE COULD CALL ^NUE DAT ARTY^ FOR CLEARANCE ALONG HIGHWAY 15, AND WAS CLEARED FOUR CLICKS WEST OF THE HIGHWAY. THE SHIP WAS A LITTLE MORE THAN TEN MINUTES OUT OF ^VUNG TAU^ WHEN THE LOW RPM WARNING LIGHT CAME ON. A QUICK CHECK OF THE INSTRUMENTS REVEALED NOTHING OUT OF THE ORDINARY. THE RPM WAS 6600 AND THE N-1 GAS PRODUCER REGISTERED 93%. EVERYTHING WAS IN THE GREEN. THE NEXT INDICATION THAT SOMETHING WAS DEFINITELY AMISS WAS A ROAR COMING FROM THE TRANSMISSION SECTION ACCOMPANIED WITH THE SMELL OF BURNING GREASE. THE PILOT THEN INSTRUCTED THE CREW TO CHECK FOR A FIRE IN THE REAR. THEY REMOVED THE AFT WINDOW FROM THE CARGO DOORS FOR A BETTER LOOK. THERE WAS NO EVIDENCE OF FIRE. NEXT THE PILOT, ^MR GILYARD^ TOOK OVER THE CONTROLS TO CHECK FOR AN ANTI-TORQUE FAILURE. THE PEDALS RESPONDED CORRECTLY. A PRECAUTIONARY LANDING WAS QUICKLY DECIDED UPON, SO THAT ALL WERE LOOKING FOR A SAFE PLACE TO LAND. ^MR GILYARD^ CALLED ^CAPITOL CENTER^ AND INFORMED THEM OF THEIR SITUATION AND THAT THEY WERE GOING TO LAND OUT AT A FIRE SUPPORT BASE SOUTHWEST OF ^BEARCAT^. ^1LT CASKEY^ CALLED ^BEAR CAT ARTY^ FOR THE COORDINATES OF THE FIRE SUPPORT BASE WHICH WERE ^YS 1289^. ^MR. GILYARD^ THEN INFORMED ^CAPITOL CENTER^ OF THEIR LOCATION. A SOUTHWEST APPROACH WAS DECIDED UPON, BECAUSE OF THE PREVAILING WINDS. WHEN THE AIRCRAFT WAS ON SHORT FINAL TO THE WEST THE PILOT BEGAN TO INCREASE POWER TO SLOW HIS RATE OF CLOSURE. WHEN THE AIRCRAFT WAS ABOUT EIGHT FEET OFF THE GROUND, THE TAIL SUDDENLY WRECHED TO THE LEFT WITH THE NOSE SPINNING TO THE RIGHT. THE AIRCRAFT TURNED ABOUT ONE HUNDRED AND EIGHTY DEGREES FROM THE DIRECTION OF LANDING. THE PILOT'S IMMEDIATE ACTION WAS TO ROLL OFF THE THROTTLE AND ATTEMPT A HOVERING AUTOROTATION WHICH RESULTED IN THE AIRCRAFT MAKING A HARD LANDING ON ITS LEFT SKID CAUSING THE MAIN ROTOR BLADES TO FLEX DOWN AND STRIKE THE GROUND ON THE LEFT SIDE OF THE AIRCRAFT THREE TIMES. THE FORCE OF THE BLADE STRIKING THE GROUND WAS ENOUGH TO FORCE THE SHIP TO RIGHT ITSELF. THE PILOT AND CO-PILOT MADE SAFE AND SPEEDY EXITS FROM THE AIRCRAFT; HOWEVER, THE CREW HAD SOME DIFFICULTY EXITING THROUGH THE OPEN AFT WINDOW OF THE CARGO DOORS.\\

Accident Summary:
THE AIRCRAFT WAS PERFORMING RESUPPLY AND MEDIVAC MISSIONS FOR THE 44TH ARVN REGIMENT. THE APPROACH INTO THE LZ WAS TO BRING RESUPPLY OF APPROXIMATELY 1000 POUNDS OF CARGO. THE PILOT MADE THE APPROACH AND TERMINATED AT A 2 TO 3 FOOT HOVER. AT THIS TIME THE CREW CHIEF AND GUNNER CLEARED THE AIRCRAFT DOWN AS THE LZ WAS COVERED WITH HIGH GRASS AND STUMPS. NOTHING UNUSUAL WAS NOTED DURING THE FINAL DESCENT AND TOUCHDOWN. THE AIRCRAFT LIFTED OFF THE LZ AND PROCEEDED TO DUC LOP FOR LUNCH AND REFUEL. UPON POST-FLIGHT INSPECTION OF THE AIRCRAFT IT WAS DISCOVERED THAT THEY HAD SET DOWN ON A STUMP. THERE WAS A HOLE IN THE UNDERSIDE OF THE FUSELAGE FORWARD OF THE REAR CROSS TUBE.\\
