Accident Summary:

DESCRIPTION OF THE ACCIDENT:\\ AIRCRAFT WAS NUMBER SIX IN A FLIGHT OF EIGHT AIRCRAFT WORKING IN THE CAI LAY AREA. BEFORE THE CRASHTHE FLIGHT WAS DIRECTED TO CARRY OUT EXTRACTION FROM ON AREA AT GRID REFERENCE XS 065611 TO A ROAD NORTH OF MY PHUOC TAY AT GRID REFERENCE XS 208595. THE FLIGHT HAD ALREADY COMPLETED TWO LIFTS FROM THIS PZ AND WERE ENROUTE TO THE THIRD LZ AT THE TIMEOF THE INCIDENT. ACCORDING TO THE STATEMENTS OF CPT MILLER AND LT MAYO THE AIRCRAFT WAS AT MAX GROSS OR VERY CLOSE TO IT. IT TOOK 43 LBS OF TORQUE TO MAKE THE TAKEOFF AND THE TAKE OFF WAS A THREE INCHE RUNNING TYPE. THE AIRCRAFT WAS JOINING UP WITH THE FLIGHT WHEN THE PILOT HEARD AND FELT A LOUD BANG. THE AIRCRAFT YAWED NOSE LEFT VIOLENTLY, LT MAYO ATTEMPTED TO PUT IN RIGHT PEDAL BUT THE AIRCRAFT COMMANDER CPT MILLER, WAS ON THE CONTROLS AND ALREADY HAD RIGHT PEDAL APPLIED AND TOOK COMMAND OF THE AIRCRAFT. THE AIRCRAFT WAS THEN SET INTO A 60 KNOT AUTOROTATION ALTITUDE AND A MAYDAY CALL WAS PUT TO THE REST OF THE FLIGHT. CPT MILLER HEADED THE AIRCRAFT INTO THE WIND AND CONTINUED WITH THE AUTOROTATION. CPT MILLER STARTED TO FLARE THE AIRCRAFT AND RAN OUT OF AFT CYCLIC. THE PITCH PULLWAS MADE AT APPROXIMATELY 15 FEET FROM IMPACT AND THE AIRCRAFT HIT HARD ON A HEADING OF 010 DEG. THE CREW EXITED THE AIRCRAFT WITHOUT ANY PROBLEM.\\
