Accident Summary:

AIRCRAFT 67-17492 DEPARTED BEAR CAT ON A ROUTINE TEST FLIGHT TO CHECK THE AIRCRAFT RADIOS. THIS WAS NECESSARY BECAUSE THE RADIOS HAD BEEN WRITTEN UP FOR DEFICIENCIES IN FLIGHT. THE AIRCRAFT DEPARTED BEAR CAT TRAFFIC PATTERN OUT THE WESTERN ACCESS ROAD AND TURNED SOUTH TOWARD THE SAIGON RIVER. APPROXIMATELY 15 MINUTES AFTER TAKE OFF THE AIRCRAFT SUDDENLY SWERVED TO THE LEFT, NOSED OVER AND STARTED SPINNING RAPIDLY TOWARD THE GROUND. ALTITUDE AT THE TIME OF THE EMERGENCY WAS BETWEEN 1000 AND 1500 FEET WITH AIRSPEED OF ABOUT 90 KNOTS. THE PILOT MADE SEVERAL CORRECTIONS TO TRY TO BRING THE AIRCRAFT UNDER CONTROL AND FINALLY GOT IT INTO A LEVEL ATTITUDE AND THE AIRCRAFT CONTINUED TO SPIN. PRIOR TO IMPACT THE PILOT ROLLED THE THROTTLE OFF AND APPLIED COLLECTIVE IN ORDER TO CUSHION THE FALL. THE AIRCRAFT STRUCK THE WATER RELATIVELY HARD AND ROLLED ONTO THE RIGHT SIDE ALMOST COMPLETELY SUBMERGED IN A CANAL. ALL PERSONNEL ESCAPED WITH ONLY MINOR INJURIES AND WERE PICKED UP IN A SAMPAN LATER. TRANSFERRED TO A HELICOPTER AND RETURNED TO BEAR CAT.\\
